Error 2227 - cannot restore item back to outlook

I have a situation where I try to restore an item from outlook, icon turns to pending restore, and goes back to archive state.

this only happens on one specific user, followed by an event in the server:

Event ID: 2227 User '<unavailable>' failed to restore an item into mailbox 'Ilia')'. |SavesetID: 201306040319818~201304281326580000~Z~214CFD249872FD4CAE023BAF6A5AF1A1

If I try to restore from archive explorer, instead of user '<Unavailable>' I will get User '<Ilia>' failed to restore an item into mailbox 'Ilia')'. |SavesetID: 201306040319818~201304281326580000~Z~214CFD249872FD4CAE023BAF6A5AF1A1

but same error.

tried to:

zap the mailbox.

clear cache of user.

clear outlook profiles.

adding dtrace of RetrievalTask.
