Clearwell Loadfile/opticon example

Hello, was curious if anyone out there had a minimum format example they give out or use for loadfiles to load into clearwell.  After some issues with a few loadfiles produced to us, and working with Clearwell tech support, it appears their guide leaves out the fact that Clearwell needs a filename in the .csv/.dat file...not just for loose/attachment files.  Hope that makes sense?
